A health risk assessment (HRA) is a short, confidential survey designed to assess your true health status. Find out the following when you complete the HRA:

"Healthy age" (how old you really are based on your current lifestyle habits)

Things you can do to improve your health
Please allow 20-30 minutes to complete the survey. It is important to answer as many of the questions you can. Please have your blood pressure and cholesterol results handy. The more questions you complete, the more accurate your wellness score will reflect the true "you." The online HRA will give you an instant report. The survey is also available in paper. If you would like our paper HRA, please contact MercyCare Customer Service at 800-895-2421. You will be sent an HRA survey and consent. Please return it in the postage paid envelope. You will receive your report in the mail in about 2 weeks.
