If you need to set up an HRA account with MercyCare, please contact mercycarequality@mhemail.org for assistance for new account activation. You will need to provide the following information: first name, last name, date of birth, and sex (if not clear).
Health risk assessment
A health risk assessment (HRA) is a short, confidential survey designed to assess your true health status. Find out the following when you complete your HRA:

Stress and substance use

Safety

"Health age" (how old you really are based on your current lifestyle habits)

Things you can do to improve your health
Please allow 20-30 minutes to complete the survey. It is important to answer as many of the questions you can. Please have your blood pressure and cholesterol results handy. The more questions you complete, the more accurate your wellness score will reflect the true you. The online HRA will give you an instant report.
